PDMP Notification to Patient

The Oregon Prescription Drug Monitoring Program (PDMP) is a program to help health care providers and pharmacists provide better patient care in managing patients’ prescriptions. They can access the program database to view more complete patient prescription information. Information about this prescription will be reported to the PDMP by the pharmacy as required by state law ORS 431.964. Prescription Readers and Language Services Policy

As required by OAR 855-041-1131 and OAR 855-041-1132, this pharmacy provides, upon request,  prescription readers for visually impaired patients. A prescription reader is a device designed to audibly convey labeling information. Upon request, this pharmacy also provides oral interpretation and translation services for any patient who is of limited English proficiency, for the following languages:  Spanish, Russian, Somali, Arabic, Chinese (simplified), Vietnamese, Farsi, Korean, Romanian, Swahili, Burmese, Nepali, Amharic, Pashtu. When dispensing a drug, the pharmacy will provide labels and information inserts in both English and in one of the languages stated above.
